Payment & Office Policies

Copays are due at the time of service

If insurance information is not available at your visit, payment will be required at that time.

Patients are responsible for understanding their own insurance coverage

This includes referrals, authorizations, deductibles, and covered services.

Cancellations & No-Shows

Appointments canceled with less than 24 hours’ notice may be subject to a $25 fee.
