A provider-patient relationship is established only when the Practice affirmatively accepts you as a patient and an appropriate clinical encounter occurs under applicable law and practice policies.

6. “Hormones Done Right” Service Line
The phrase “Hormones Done Right” is used on this website as a branding, marketing, or service-line descriptor associated with hormone-focused services offered through or in connection with Guadalupe Valley Women’s Health Care Center. Unless and until separately established and disclosed, “Hormones Done Right” is not a separate legal entity, separate medical practice, or separate licensed healthcare provider.
All medical services are rendered, if at all, through the applicable legally operating entity and licensed clinicians.
7. Telehealth Services
Certain services may be offered through telehealth where clinically appropriate and legally permitted. Telehealth availability is subject to provider discretion, patient location, technology limitations, scheduling availability, licensure requirements, and clinical appropriateness.
Use of this website does not guarantee that telehealth will be offered, appropriate, or available for your needs. Certain complaints, examinations, treatments, prescriptions, diagnostics, procedures, and follow-up recommendations may require an in-person visit. Texas law provides that telemedicine is subject to the same standard of care as an in-person encounter.
8. Geographic and Licensure Limits
Medical services, including telehealth services, are offered only where our clinicians are authorized to practice and where such services may lawfully be provided. You are responsible for accurately disclosing your identity, age, location, and contact information when requesting care.
We reserve the right to decline services if you are located in a jurisdiction where we do not provide care, where licensure or legal restrictions apply, or where telehealth is not clinically appropriate.
9. No Guarantee of Prescription, Testing, Treatment, or Results
Nothing on this website guarantees that any clinician will prescribe medication, order labs, order imaging, continue prior therapy, recommend hormone treatment, refill medication, or agree with any prior diagnosis or treatment plan.
All clinical decisions are made at the sole discretion of the treating clinician based on medical judgment, applicable law, patient safety, available information, and practice policies.
The Practice does not guarantee any particular outcome, improvement, symptom relief, diagnosis, treatment plan, or patient result.
10. Hormone, Wellness, and Symptom-Related Content
Content regarding hormones, menopause, perimenopause, wellness, fatigue, sexual health, mood, cycle concerns, weight-related symptoms, or related topics is intended for general education only. Individual symptoms can have multiple causes, some of which may be serious.
Website descriptions of hormone-related care should not be interpreted as promises of symptom reversal, disease treatment, improved vitality, improved sexual function, prevention of aging, guaranteed weight loss, or other specific benefits unless expressly supported and clinically appropriate in an individualized encounter.

Online scheduling, appointment requests, pre-registration, and form completion are convenience features only. Submission of a request does not guarantee acceptance as a patient, appointment confirmation, or treatment.
Appointments are not final until confirmed by the Practice.
We reserve the right to reschedule, cancel, refuse, or limit appointments for administrative, legal, clinical, safety, or operational reasons.
12. Patient Portal and Electronic Communications
As of posted date, this website does not link to a patient portal. In the future, if this website links to a patient portal or secure communication platform, separate portal terms, privacy terms, and patient agreements may apply. Portal use may require account credentials, identity verification, and compliance with specific instructions.
You would be responsible for maintaining the confidentiality of any login credentials and for restricting unauthorized access to your device or account.
13. Text Messaging, Email, and Contact Forms
